Request for a Revision to Financial Aid Package and Budget
Students may have expenses greater than those reflected in the estimated student expenses portion of their Financial Aid Award Letter. The Office of Student Financial Aid and Scholarships recognizes that some situations, if thoroughly documented, justify additional funding within the limits of federal and institutional policy. Students also may wish to revise their aid based upon part time enrollment or update their grade level status mid-year.

Satisfactory Academic Progress Petition
Students who have been notified by the Office of Student Financial Aid and Scholarships that they are ineligible to receive financial aid due to not making progress towards graduation are required to obtain an approved Satisfactory Academic Progress Petition in order to receive financial aid at the University of Oregon in the future.

Verification Worksheets
If you have been selected for a process called verification we have requested that you submit a Verification Worksheet along with appropriate federal tax documents. Below are the worksheets you may download for your use.
Please make sure you complete the worksheets and provide all necessary signatures before you submit it to the financial aid office. Incomplete forms will be returned to you and may cause a delay in processing your aid application.
The IRS Topic 56 - Copy of Your Tax Return - How to Get One provides a link to Form 4506-T. The free transcript of your tax return maybe used to satisfy the tax form requirement of the verification worksheets. You are required to sign the transcript before submitting it to our office.

Graduate PLUS Application Form
Complete the information on the Graduate PLUS Application Form included with your student’s Financial Aid Award Letter packet. Make a copy for your records, and return the Graduate PLUS Application Form to the Office of Student Financial Aid and Scholarships.
A copy of the form without a certified loan amount is available here. Contact the Office of Student Financial Aid and Scholarships to obtain the maximum eligibility of the certified loan amount for the Graduate PLUS.
